Following years of extremely challenging industry conditions and operating losses, it is with deep regret that the Board of Directors has made the difficult decision to permanently close Clarview Nursing & Rehabilitation Center and Country Springs Personal Care Home, effective January 27, 2025.
We appreciate the trust you put into Clarview and Country Springs in caring for you or your loved one. It has been an honor to be part of this community.
Notes For Patients
The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than seven years after the last date of services you received from this Practice, although most records will be held for at least 10 years from the date of last service, depending upon applicable state and federal record retention requirements.
