Our team will work with you, your family and your caregivers to establish individual goals at the time of admission while performing avoidable hospital re-admission measures during your stay. Education and assistance with advanced care planning will be presented. Discharge education, resource coordination and follow-up support will be coordinated prior to transitioning home or to a lower level of care. After discharge, we will share your success with your physician and evaluate your satisfaction with your stay.
